    RISC Topics to Prioritize

    When study time is limited, use the ABR blueprint to cover all eight RISC areas and use your own practice results to choose what to revisit.

    Contrast Reaction Management: Review current clinical guidance for contrast reactions, including severity assessment and response principles. The ABR blueprint should guide scope; it does not establish a public frequency ranking.
    Radiation Units and Conversions: Review the relationship between gray (Gy) and sievert (Sv), effective and absorbed dose, and tissue weighting factors against current authoritative sources.
    MRI Zone Classification: Knowing what is permitted and prohibited in each zone — and the rationale behind the restrictions — is a classic RISC question category. The four-zone model and its clinical application are high yield.
    Dose Limits: Review occupational dose limits, pregnancy-related guidance, and the distinction between whole-body and extremity limits against current authoritative sources.
    NSF Risk Stratification: Which gadolinium agents carry the highest versus lowest NSF risk, and the eGFR thresholds that modify clinical decision-making, are important for both RISC and clinical practice.
    ACR Practice Parameters: The ACR has published practice parameters for communication of diagnostic imaging findings that specify timing requirements for critical results and documentation standards. These are explicitly referenced in the ABR content blueprint.

    Why RISC Catches Residents Off Guard

    RISC includes technical and safety concepts that benefit from deliberate review:

    **The content includes numerical knowledge.** RISC study can include dose limits, contrast dosing thresholds, eGFR cutoffs, and SAR limits. Check values against current authoritative guidance rather than relying on memorized third-party summaries.

    **Clinical exposure is uneven.** Clinical experience may not cover the underlying pharmacology, MRI zone model, or QC concepts in depth. Pair clinical exposure with systematic review and authoritative references.

    **The radiation physics questions require a different cognitive mode.** Converting between dose units, applying tissue weighting factors, and calculating effective dose from absorbed dose are math-adjacent tasks that feel unfamiliar to residents who haven't done them since medical school physics.

    **Communication deserves intentional review.** Include critical-results protocols and communication parameters in your coverage of the RISC blueprint rather than assuming any topic is absent or frequent.

    How to Study RISC Effectively

    A structured RISC study approach should address both the factual recall requirements and the application-level scenarios.

    **Create a fact sheet for numerical thresholds.** Write out the key numerical values — dose limits, eGFR thresholds, contrast pre-medication protocols, MRI SAR limits — and review them regularly. These are the facts most likely to be tested directly and most easily forgotten without reinforcement.

    **Work through RISC questions by category.** Don't mix RISC topics indiscriminately. Study radiation safety questions as a focused block, then contrast reactions, then MRI safety, then QC. Category-based studying builds a mental schema for each topic area that random-order practice doesn't.

    **Trace your errors to their source.** When you miss a RISC question, determine whether the error was a knowledge gap (you didn't know the fact) or a reasoning error (you knew the facts but applied them incorrectly). These require different remediation strategies.
